Do you need vaccinations?
If you require any vaccinations relating to foreign travel you need ask at reception for a travel vaccination request form. The form can be completed over the telephone . You will need to provide the following information . It is vital that you give full details of destinations and resorts as vaccination requirements can vary from resort to resort even within the same country.

- Length of stay
- Holiday Type
- Accommodation e.g. hotel, self catering , staying with family
- High Risk activities e.g scuba diving safari

Once you have completed the form you will be asked to contact the surgery in a few days once the nurse has reviewed your request and worked out you vaccination programme. When you ring back for this information the receptionist will provide you with the information about what you need and advise you as to what appointments are required.
We ideally like to start a vaccination programme 4-6 weeks before departure .
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

We offer a full travel advice and immunisation service. If you are travelling abroad please complete and submit the travel form in plenty of time (we recommend 6-8 weeks) before your journey.

Travel Vaccination Prices
Travel advice is covered by the NHS; however some vaccinations and prescriptions are only available privately. Payment for non-NHS services must be made at the first travel appointment.
